Sliding Scale Fee Payment Program

Ivy’s Healing Path Mental Health Services LLC is committed to improving access to care. A limited sliding scale fee program may be available for patients experiencing financial hardship who do not have insurance coverage.

Sliding scale appointments are offered based on income, household size, and availability. Documentation may be requested to determine eligibility. Participation in the sliding scale program is not guaranteed and is subject to clinic capacity.

If you believe you may qualify, please indicate your interest during the intake process and our office will review your request.

Start Your Journey to Wellness – New Patient Form

To help us provide the best possible care from the start, all new clients are asked to complete a brief intake form before their first appointment. This form collects important information about your medical history, current concerns, and treatment goals. Completing it in advance ensures your provider has the context needed to create a personalized, effective care plan. You can fill it out securely online.
